Skilled Nursing Services
Our skilled nursing services are performed by a licensed nurse and can include wound care, injections, tube feedings, disease management, medication management, catheter changes, observation and assessment of your condition, management and evaluation of your care plan, and patient/caregiver education. Your nurse will coordinate your plan of care with your medical team.
Our physical therapists will partner with you to help you regain mobility. Therapy services can include modifying your home for safety and fall prevention, helping you build strength and endurance with in-home exercises, and much more.
When recovering from a surgery, illness or injury, you may need help learning how to complete activities of daily living again such as eating, grooming and bathing. Our trained therapists also assess cognitive function and implement measures to maintain or improve function. Therapists make recommendations for home modifications to promote independence and safety.
Our therapists are trained to help you with speaking, eating, breathing, swallowing, cognition skills and communication disorders. Our caregivers also assess cognitive function and implement measures to maintain or improve function.
Medical Social Workers
Our medical social workers help you navigate the complexity that often comes with managing home health care – and beyond. Our team members will help you determine your needs, guide you to beneficial community resources and support, educate you and your family members, and help you with long-term planning upon discharge from our home health services.
Home Health Aides
Our experienced home health aides help you regain independence by lending support with activities of daily living, such as eating, bathing, getting dressed, toileting, transferring and continence. We carefully select each aide to ensure he/she brings a caring and empathetic approach to every visit.
Personal Care & Private Duty Services
Our compassionate caregivers understand that normal day-to-day activities can become more challenging after an illness, injury or surgery, and as we age. We get to know you and your needs to determine what services are most beneficial. Support can include housekeeping services, meal preparation, transportation, grooming and hygiene, companionship care, 24-hour care, respite care, medication reminders, overnight assistance, and much more.
Our experts will help you set up a Lifeline alert service that ensures you have fast, reliable access to a care response team at the push of a button. From a medical emergency, fall, fire, or being locked out of your house, you'll have access to help 24/7. And, our team of caregivers monitor reports and will follow up with you within 24 hours to determine if our caregivers can provide any additional assistance or training.
We understand that managing multiple medications can be challenging, so our caregivers are trained to help ensure proper doses are taken at the correct times, help answer any questions you have, and help manage your medication ordering and tracking. We also offer medication dispensers to safely provide increased independence.
